Leave of Absence

Effective Feb 27, 2012, Dr. Eric Bodenstab will be on indefinate leave of absence.

- all patients who have appointments with Dr. Bodenstab are being re-booked with Dr. Prystupa for the month of March 2012 

- patients calling in to book an appointment date in March with Dr. Bodenstab can book with Dr. Prystupa.

We appologise for any inconvenience this may cause.

Attention: All Patients

As of January 31, 2012 patients will be billed directly for all services if they do not have the 2014 renewal sticker on their health card.

Please call Saskatchewan Health at 1-800-667-7551 if you have not received your renewal sticker.

Notice to All Patients of Dr. Aaron Prystupa

Dr. Aaron Prystupa will be moving back to North Battleford.  His last day of practice at Lakeside Medical Clinic will be March 30, 2012.

Please check with reception to schedule an appointment after this time with one of our physicians accepting patients.

Patients from North Battleford may contact North Battleford Medical Clinic for details of his return.

Patient Information Collection and Verification

It is very important that we have your correct contact information (date of birth, address, phone numbers and Next of Kin contact) on file as well as verifying valid health care coverage.  In an effort to verify this information as well as satisfy policies in the Health Information Protection Act, effective February 1st, our Reception staff will be asking you all this pertinent information when you call in to book your appointment .... instead of when you check-in on the day of your appointment.  Thanks for your co-operation and understanding.
